Additionally, we offer temporary residential care for those with medical or behavioral health needs but without stable housing. Employing a scattered site housing model, rooted life ensures each residence is fully furnished, creating environments conducive to healing and recovery. Responsibilities:

What you'll be doing:

• • Customer service excellence: providing the highest level of service to both internal and external customers in a fast- aced, team- oriented environment.
• • Active intake calls: making the assigned number of intake calls and collaborating with the operations manager to reach team goals
• • Referral process management: managing the total referral process, including initiation, patient, and insurance verification, as well as assignment of referrals and record- keeping.
• • Service determination: determining services that referrals may require.
• • Client connection: ensuring clients are connected to appropriate staff and the services necessary to meet their needs.
• • Seamless referral collaboration: collaborating with other staff and community service providers to ensure seamless referral services.
• • Efficient prioritization: prioritizing and working efficiently throughout the workday with a strong attention to detail.

Qualifications & requirements:

• • 2 years minimum experience as an intake coordinator
• • Strong organizational, communication, and interpersonal skills.
• • Proficiency in computer applications and databases.
• • Excellent data entry and typing skills
• • Fluent english
